Job summary

Signify Health, part of CVS Health, seeks per diem Nurse Practitioners or Physician Assistants to perform in-home health evaluations. You will connect with members in their homes, review medical history, conduct physicals, and perform tests as ordered by health plans.

Travel locally and occasional virtual visits are expected in a flexible, part-time role. Active, unrestricted licenses with board certification are required; multi-state licenses are a plus.

• Active, unrestricted license in coverage area(s).
• Board certification is required.
• Multi-state licenses preferred.

Responsibilities

• Provide in-home health evaluations for plan members.
• Conduct physical evaluations and medical history reviews.
• Perform required diagnostic tests when ordered by health plan.
• Travel locally to member homes and conduct virtual visits as needed.
• Use portable devices to document findings and communicate with care teams.
• Adhere to Signify Health protocols and maintain patient safety.
