Job summary

Signify Health, part of CVS Health, is seeking a Per Diem Clinician (NP or PA) to perform in-home health evaluations. This part-time role involves visiting members at home, reviewing medical history, and conducting physical evaluations using an iPad workflow.

Travel locally and occasional virtual visits are required, with a base hourly pay and incentive structure. Qualified candidates hold active NP or PA licenses with board certification; multi-state licenses are a plus.

• Active, unrestricted license(s) in coverage area(s).
• Board certification required.
• Multi-state licenses preferred.

Responsibilities

• Perform in-home health evaluations for individuals at their residences.
• Engage with patients to review medical history and perform physical assessments.
• Coordinate with health plans and Signify Health services as indicated.
• Travel locally to member homes and conduct virtual visits as needed.
• Provide diagnostic and preventive services per program scope.
