Job summary

Healogics, LLC. is seeking an experienced Licensed Practical Nurse (LPN/LVN) to join our Wound Care Center in Oklahoma City.

You will provide wound care and related nursing support under the direction of an RN or provider, following hospital and CMS guidelines, and you may assist in the vein clinic after training. Required are a state LPN/LVN license, graduation from an accredited program, and 2+ years of LPN/LVN experience, with wound care experience preferred.

Qualifications

• High School Diploma or GED.
• Graduate from an accredited practical nursing program/state approved educational program.
• Current LPN/LVN license in the state of practice and maintained throughout employment.
• Two or more years’ experience as LPN/LVN, including SNF or LTACH setting.
• Wound care experience preferred.

Responsibilities

• Under RN/Provider direction, delivers wound-related patient care per BLS and hospital protocols.
• Assist in vein clinic as trained and assigned.
• May assist with intake and discharge.
• Support case management under RN direction and state rules.
• Gather patient information and equipment for care.
• Report care quality issues to the Clinical Nurse Manager.
• Communicate with team on interventions to ensure quality care.
• Possibly function as a scribe per policy.

Education

High School Diploma or GED

LPN/LVN license

Accredited practical nursing program

2+ years LPN/LVN experience

Wound care experience preferred
