Qualifications

• Current LPN license in the State of New Hampshire.
• Long-term care or skilled nursing experience preferred.
• Strong clinical, organizational, and communication skills.

Responsibilities

• Provide direct nursing care in accordance with care plans and physician orders.
• Administer medications and treatments safely and accurately.
• Monitor residents' conditions and report changes to the RN or physician.
• Document care and maintain accurate clinical records.
• Supervise LNAs and support team-based care delivery.
• Promote residents' dignity, comfort, and overall well-being.
• Follow infection control, safety, and regulatory guidelines.
