Job summary

The Paramount at Somers is seeking a Licensed Practical Nurse (LPN) to join our Skilled Unit and Long Term Care Unit teams in Somers, NY. This per diem role requires a current LPN license and involves direct patient care, medication administration, vital signs, and family education.

Responsibilities include supervising CNAs, coordinating with physicians and NPs, and ensuring high-quality, compassionate care for residents. Per diem shifts offer flexibility for experienced LPNs.

Qualifications

• Must hold current LPN license and be in good standing.
• Nursing license required for New York state.

Responsibilities

• Provide care for residents in SNF and Long Term Care Unit.
• Administer medications, including IVs where applicable.
• Take and record vital signs accurately.
• Provide resident care and treatments with monitoring of outcomes.
• Educate residents and families about care plans.
• Communicate with physicians, NPs, families and other health care professionals.
• Supervise CNAs to ensure quality care.
