Job summary

The Paramount at Somers is seeking aLicensed Practical Nurse (LPN) on a per diem basis to provide skilled nursing care at our Somers, NY facility. You will work in the Skilled Unit and Long Term Care Unit, delivering medications (including IVs), monitoring vitals, and supporting residents and families.

Applicants must hold a current LPN license and be prepared to collaborate with physicians, NPs, and the care team, while supervising CNAs and educating residents and families about care plans.

Qualifications

• Current LPN license is required.

Responsibilities

• Provide care for residents of the Skilled Unit (SNF) and Long Term Care Unit.
• Medication administration, including IVs.
• Take and record vital signs.
• Provide resident care and treatment with monitoring of outcomes.
• Provide resident and family education.
• Communicate with physicians, NPs, families and other health care professionals.
• Supervise CNAs.
