The Paramount at Somers is hiring a Licensed Practical Nurse (LPN) for per diem shifts at our Somers, NY facility. You will provide resident care in the Skilled and Long Term Care Units, administer medications (including IVs), monitor outcomes, and educate residents and families.

Collaboration with physicians, NPs, and other health professionals is essential, and CNAs are supervised. Current LPN license is required.

Qualifications

• Current LPN license.

Responsabilités

• Provide care for residents of our Skilled Unit (SNF) and Long Term Care Unit.
• Medication administration, including IVs.
• Taking and recording vital signs.
• Providing resident care and treatments with monitoring of outcome.
• Provide resident and family education.
• Communication with physicians, NPs, families and other health care professionals.
• Supervising CNAs.
