Job summary

The Paramount at Somers in Somers, NY is seeking a licensed practical nurse (LPN) to join our skilled and long-term care teams. You will administer medications, monitor residents, take vital signs, and provide education to residents and families while collaborating with physicians and other care professionals.

Qualifications

• Current LPN license valid in New York.
• Experience in skilled or long-term care preferred.
• Ability to monitor outcomes and document accurately.

Responsibilities

• Provide care for SNF and Long Term Care Unit residents.
• Medication administration including IVs.
• Take and record vital signs.
• Provide resident education and monitor outcomes.
• Educate residents and families as needed.
• Communicate with physicians, NPs, families and other health professionals; supervise CNAs.
