Job summary

Redmond Assured Home Health, part of LHC Group and a member of the Optum family, seeks a Per Diem LPN/LVN to deliver quality patient care in patients' homes. You will follow the plan of care, coordinate with the health care team, and support families in home environments.

Qualified applicants hold Washington LPN/LVN licensure, a valid driver's license, and CPR certification or the ability to obtain it within 90 days of hire.

Qualifications

• Current and unrestricted LPN or LVN licensure in the state of Washington.
• Current driver's license, vehicle insurance, and access to a dependable vehicle or reliable transportation.
• CPR certification or ability to obtain within 90 days of hire.

Responsibilities

• Provides services within the scope of practice as defined by state laws governing the practice of nursing, in accordance with the plan of care, and in coordination with other members of the health care team.
• Instructs the patients family in the care of the patient and maintenance of a healthy environment for the patient.
• Observes, records, and reports to the supervising nurse and/or physician the patient's response to treatment and changes in the patient's condition daily.
• Ability to function in any home situation regardless of age, race, creed, color, sex, disability, or financial condition of the client
