Optum is seeking a Per Diem Licensed Practical Nurse (LPN) or Licensed Vocational Nurse (LVN) to provide high quality home health care in patients' homes. You will work within the plan of care, coordinate with the health care team, and educate families to maintain a healthy environment.

Qualifikationen

• Current and unrestricted LPN or LVN licensure in state of practice.
• Current Driver's License, vehicle insurance, and access to a dependable vehicle or public transportation.
• CPR certification or ability to obtain within 90 days of hire.
• Good communication, writing, and organizational skills.

• Provides patient care within the scope of practice and plan of care.
• Instructs the patient’s family in care and healthy environment maintenance.
• Observes, records, and reports patient responses to treatment and condition changes daily.
