Job summary

Elite Home Health, part of the LHC Group and UnitedHealth Group family, is hiring a Per Diem LPN/LV N to provide in-home patient care two days a week. You’ll work within a care plan, educate families, and monitor patient responses in diverse home environments.

Qualified candidates hold an active LPN or LVN license, have a valid driver’s license, and can flex hours. CPR certification and home health experience are preferred to ensure high-quality, compassionate care.

Qualifications

• Current and unrestricted LPN/LVN licensure in state of practice.
• Driver's license and access to a dependable vehicle or public transportation.
• CPR certification or ability to obtain within 90 days.
• Home care experience preferred.
• Ability to work flexible hours and independently.
• Strong communication, writing, and organizational skills.

Responsibilities

• Provides services within the scope of practice as defined by the state laws governing the practice of nursing, in accordance with the plan of care, and in coordination with other members of the health care team.
• Instructs the patient’s family in the care of the patient and maintenance of a healthy environment for the patient.
• Observes, records, and reports to the supervising nurse and/or physician the patient’s response to treatment and changes in the patient’s condition daily.
• Ability to function in any home situation regardless of age, race, creed, color, sex, disability, or financial condition of the client.
