Job summary

Elite Home Health, part of LHC Group in the Optum family, is seeking a Per Diem Licensed Practical Nurse (LPN) or Licensed Vocational Nurse (LVN) to deliver quality in-home patient care. You will play a vital role in providing skilled care under a plan of care, coordinating with the health care team and educating families.

Qualifications

• Current and unrestricted LPN or LVN licensure in state of practice
• Current Driver's License, vehicle insurance, and access to a dependable vehicle or public transportation
• CPR certification preferred or ability to obtain within 90 days of hire

Responsibilities

• Provides services within the scope of practice as defined by state laws and plan of care, and coordinates with the health care team
• Instructs the patient’s family in care and healthy home environment
• Observes, records, and reports patient responses to treatment to the supervising nurse/physician
• Ability to work in any home setting regardless of patient demographics
