Job summary

Monarch Communities is seeking a Wellness Nurse (LPN) to provide direct nursing care to residents with an emphasis on holistic wellness. This per-diem role reports to the Health and Wellness Director (RN) and includes supervising day-to-day nursing activities by HHAs.

You will cover multiple shifts in a Connecticut facility, ensuring safety, compliance with state regulations, and compassionate care for residents including those with dementia.

Qualifications

• Graduation from an accredited School of Nursing with current LPN license.
• Minimum 2 years of experience in long-term care, assisted living, home health, or hospital setting preferred.
• Experience with residents with Alzheimer's or related dementias.
• Ability to handle multiple tasks and learn nursing practices and regulations.
• Proficiency in Microsoft Office and ability to learn new applications.

Responsibilities

• Monitor health, safety, and well‑being of residents.
• Assist in training and monitoring of medication administration.
• Supervise care staff in line with regulations and policies.
• Maintain clinical quality assurance per standards.
• Promote high-quality service and embody the company mission.
• Conduct thorough resident assessments.
