Job summary

Monarch Communities is seeking a Wellness Nurse (LPN) to provide direct nursing care for residents, with supervision of care staff. This per-diem, non-exempt position offers full-shift coverage and the opportunity to contribute to holistic wellness.

The role requires an active LPN license and prior experience in long-term care or related settings. Strong computer skills and physical ability to assist residents up to 50 lbs are preferred. Flexible scheduling and on-site work are available.

Qualifications

• Graduation from an accredited School of Nursing with current LPN license.
• At least 2 years’ experience in long-term care, assisted living, home health or hospital setting.
• Experience working with residents with Alzheimer’s or other dementias.
• Proficiency in computer skills, Microsoft Office (Windows, Outlook, Excel).
• Ability to lift or carry weight up to 50 lbs. and perform physical tasks.

Responsibilities

• Monitor health, safety, and well-being of residents.
• Assist in training and monitoring medication administration.
• Supervise care staff per regulations and policies.
• Maintain clinical quality assurance per standards.
• Promote high service level and mission of Monarch Communities.
• Conduct thorough resident assessments.
