Job summary

Southcoast Health is seeking a Licensed Practical Nurse (LPN) for its Dartmouth, MA Primary Care sites. This Per Diem role covers day shifts, with travel to SPG Primary Care sites as needed to ensure patient care coverage.

The LPN will administer medications, teach patients and families, and support clinic flow under the direction of a supervisor. MA/RI LPN licensure and BLS are required, with EHR experience preferred.

Qualifications

• LPN licensure required in MA and/or RI.
• BLS certification required.
• Experience with EHR systems highly desirable.

Responsibilities

• Provide direct patient care, including medication administration.
• Educate patients and families and perform patient rooming and procedures as assigned.
• Maintain accurate patient information in the EHR and coordinate care with the clinic team.
• Communicate test results, schedules, and follow-up instructions to patients.
• Travel to SPG Primary Care sites as needed to cover staffing gaps.
