Job summary

BAYADA Home Health Care in Exton, PA is seeking a compassionate Part Time or Per Diem Licensed Practical Nurse (LPN) to join our team serving adult clients in Senior Living Communities across Chester County.

You will perform home visits, administer medications, wound care, and routine treatments while documenting care and coordinating with a multidisciplinary team. A PA LPN license and at least one year of relevant experience are required.

Qualifications

• Must be a current PA licensed LPN.
• Minimum of one year of recent LPN experience.
• Graduation from an accredited nursing program.
• Home health care experience preferred but not required.

Responsibilities

• Make home visits to clients in designated territories.
• Administer medications, perform wound care, treatments, and procedures.
• Monitor client conditions and report changes to supervisor.
• Follow up on doctors' orders and document actions.
• Perform client assessments as needed.
• Coordinate case management and care delivery.
• Accurately document observations and interventions in EMR on a tablet.
