Job summary

BAYADA Home Health Care is seeking a compassionate Licensed Practical Nurse (LPN) for a Per Diem role to service adult clients in Annandale, VA and surrounding areas.

Requirements include a current VA LPN license and at least one year of LPN experience; Medicare Home Health exposure is preferred. You will perform home visits, administer medications, document orders, and coordinate care with a multi-disciplinary team.

Qualifications

• Active LPN license in Virginia.
• Minimum of one year of LPN experience.
• Home health experience preferred, not required.
• Ability to travel to cases as assigned.
• Strong communication and organizational skills.

Responsibilities

• Make home visits to clients in designated territories.
• Administer medication, perform wound care and treatments.
• Monitor client conditions and report changes to supervisors.
• Document orders, assessments and interventions in EMR.
• Coordinate case management and patient care activities.
