BAYADA by Bayhealth Home Health Care is seeking a compassionate Licensed Practical Nurse (LPN) to support adult clients across home visits in Dover, DE and surrounding areas.

The role requires a DE LPN license and at least one year of LPN experience, with opportunities for full-time or part-time schedules. Travel to cases is required as part of delivering high-quality home health care.

Qualifications

• Requires a current LPN license in DE.
• Minimum of one year of recent, verifiable LPN experience.
• Graduation from an accredited nursing program as indicated by transcript or diploma.
• Home health experience preferred.
• Ability to travel to cases as assigned.
• Strong interpersonal and time-management skills.

Responsibilities

• Make home visits to clients in designated territories.
• Administer medications, wound care, treatments, and procedures.
• Monitor clients' conditions and report changes to Clinical Manager and Client Services Manager.
• Follow up with, execute, and document doctors' orders.
• Perform client assessments as necessary.
• Case management and coordination.
• Accurately document observations, interventions, and evaluations in EMR.
