Job summary

BAYADA Home Health Care in Delaware County seeks an LPN to join our skilled nursing team providing private duty home care. This role focuses on one-on-one client care, following physicians’ orders, and documenting care in the client’s records.

Travel to client homes and performing ongoing assessments are key parts of the day, with orientation and training provided. Applicants must graduate from an accredited nursing program and hold a current Pennsylvania nursing license.

Qualifications

• Graduation from an accredited and approved nursing program.
• A current nursing license in good standing in the state of PA.

Responsibilities

• Travel to one client home near you on a schedule you choose.
• Follow up with, execute, and properly document doctors’ orders.
• Perform assessments and monitor clients’ conditions.
• Document observations, interventions, and evaluations.
