Qualifications

• Completes assessments per scope of practice and unit standards.
• Addresses patient and family physical, psychological, social, and cultural needs.
• Develops and implements plan of care with interdisciplinary team.
• Provides direct patient care including treatments and medications.
• Coordinates discharge planning and patient education.
• Collaborates with physicians and staff to meet patient needs.
• Maintains quality and safety through evidence-based practices.
