Job summary

Select Medical is seeking a PRN Case Manager at Select Specialty Hospital (LTACH) in Erie, PA, requiring licensure in nursing, respiratory therapy, or social work. Qualifications

• Current licensure in a clinical discipline (Nurse or Respiratory Therapist) OR Social Work SW/MSW per state guidelines.
• Previous RN/LPN/RT/SW/CM inpatient hospital experience.
• Experience in acute medical case management and directing a plan of care.

Responsibilities

• Develops and implements a patient-specific discharge plan.
• Performs verification of utilization criteria reviews.
• Coordinates with payor sources to assure proper reimbursement.
• Ensures compliance with Utilization Management policies and licensure/accreditation requirements.
• Maintains fiscal responsibility and maximizes insurance benefit coverage for the patient.
• Facilitates multi-disciplinary team meetings including physicians, nurses, RTs, and rehab therapists.
