Select Medical's Select Specialty Hospital in Fort Myers is seeking a PRN Case Manager for our Critical Illness Recovery Hospital (LTACH). The role focuses on utilization reviews, discharge planning, and care coordination to support chronically critically ill patients.

You will work with physicians, nurses, respiratory therapists, and rehabilitation therapists to optimize patient outcomes while ensuring compliant documentation and reimbursement.

Qualifikationen

• Current licensure in a clinical discipline as a Nurse (RN/LPN/LVN) or Respiratory Therapist or Social Work SW/MSW per state guidelines.

Aufgaben

• Responsible for utilization reviews and resource management.
• Conducts discharge planning and treatment plan management.
• Manages financial aspects and completes medical record documentation.
• Reports to the Director of Case Management and provides social work services as needed.
• Coordinates with payors to ensure appropriate reimbursement.
• Facilitates multi-disciplinary team meetings with physicians, nurses, RTs and rehab therapists.
