Job summary

Soleo Health is seeking a Per Diem Infusion Nurse to support infusion patients in Eureka, CA and surrounding areas. You will provide direct nursing care, including assessments, care planning, education, and IV therapy administration in home settings, coordinating with pharmacists, prescribers, and care partners.

The role requires California RN licensure, at least 3 years nursing experience with infusion therapy preferred, on-call duties, and a willingness to travel up to 60 miles.

Qualifications

• RN degree required; BSN preferred.
• Minimum 3 years nursing experience; infusion therapy preferred.
• On-call rotation, nights and weekends included.
• Current California RN license.
• Proficient with Outlook and Office Suite.

Responsibilities

• Provide direct patient care for assigned infusion patients in home or alternate site setting.
• Initiate and maintain communication with care team members (pharmacists, prescribers, dietitians, care partners).
• Educate patient and care partner to safely administer home infusion therapy.
• Assess home environment and identify safety interventions for prescribed care.
• Develop and maintain plan of care and evaluate response to therapy per care matrices.
