Job summary

Soleo Health is seeking a Per Diem Infusion Nurse to support infusion patients in the Raleigh, NC area and surrounding regions. You will provide direct patient care, educate patients and care partners, and coordinate home infusion services with the care team.

The role requires an active RN license in NC, BSN preferred, and infusion therapy experience; on-call rotations, driving to patient homes, and strong documentation are part of the responsibilities. Mileage reimbursement is provided.

Qualifications

• Current RN license by the applicable state’s Board of Nursing, in good standing.
• RN degree, bachelor’s degree in nursing (BSN) from an accredited institution preferred
• Advanced certification in specialty areas of practice (e.g., CRNI, ONS, IgRN) preferred but not required.
• Participation in an on-call rotation, nights and weekends included, providing as-needed support outside of regular business hours to address urgent issues and ensure continuous operations.
• Administrative Oversight: Responsible for providing administrative oversight, including monitoring of workflows, ensuring compliance with policies, and managing resources efficiently to meet organizational objectives.
• Minimum 3 years nursing experience with background in infusion therapy preferred.
• Experience in ICU and/or Cardiac Care preferred.
• Proficient with Microsoft Outlook and Microsoft Office Suite.

Responsibilities

• Provide direct patient care for assigned patients including initiation and maintenance of communications with the care team.
• Educate patient/care partner for safe home infusion therapy and provide additional instruction.
• Evaluate home environment and implement safety interventions for prescribed care.
• Develop and evaluate plan of care with other team members for each encounter.
• Manage schedule via company platform and communicate changes promptly.
• Coordinate patient care via approved company platform.
• Participate in on-call rotation to ensure continuous nursing support.
• Document all patient encounters and care activities per policy and regulatory requirements.
• Maintain knowledge of Soleo therapies, protocols, and clinical standards.
• Ensure timely submission of POT and required clinical documents per policy and state guidelines.
• Collaborate with departments to complete patient documents (consents, POTs) within required timeframes.
