Job summary

Soleo Health Inc. is seeking a Per Diem Infusion Nurse to support infusion patients in Circleville, OH and surrounding areas. The nurse will deliver direct home infusion care, perform assessments, educate patients, and coordinate with the care team to ensure safe and effective therapy.

Requirements include an Ohio RN license in good standing, 2–3 days per week availability, and ability to drive to patient homes within 60 miles as needed. Per diem hours and mileage reimbursement are available.

Qualifications

• Current RN license by Ohio Board of Nursing in good standing.
• RN degree, BSN preferred.
• Infusion therapy knowledge and/or certification preferred.
• Experience with home infusion care and coordination.
• Strong communication with care team and patients.
• Ability to operate a company platform for scheduling and documentation.

Responsibilities

• Provide direct infusion nursing care including assessments, planning, and education for home infusion patients.
• Coordinate with pharmacists, prescribers, dietitians, and home care partners.
• Evaluate home environment for safety and appropriateness of care.
• Develop and evaluate plan of care with the team and patient response to therapy.
• Manage schedule and communicate changes to the team.
• Document patient encounters and care activities at the time of service.
• Maintain knowledge of Soleo therapies, protocols and standards.
• Ensure timely submission of plans of treatment and required documents.

The Position:

The Infusion Nurse provides primary patient care for assigned patients including assessments, care planning, evaluation, and education for patients receiving infusion therapy and/or related nursing services in accordance with the care matrices and best practices in the home or alternate site setting. Responsibilities include:

• Provide direct care for all assigned patients including initiation and maintenance of all communications with appropriate care team members including pharmacists, prescribers, dietitians, home patient representatives/care partner and any appropriate community resource staff members.
• Evaluate and educate patient/care partner to safely administer the home infusion therapy and provide additional instruction as indicated.
• Evaluate the home environment where the care is to be provided and identify interventions that may be required to render the home care environment safe and appropriate for the prescribed care.
• Develop in conjunction with other team members, the plan of care and evaluate response to therapy with each encounter.
• Manage schedule via company approved platform per company policy. Communicate any scheduling changes promptly to appropriate team members.
• Coordinate patient care via approved company platform.
• Document all patient encounters, communications, and care activities at time of service as appropriate per Soleo policy and regulatory requirements.
• Maintain strong knowledge of Soleo therapies, protocols, and clinical standards.
• Ensure timely submission of plans of treatment (POT) and required clinical documents per policy and state guideline.
• Work in conjunction with appropriate departments to ensure all required patient documents (i.e. consents, POTs) are completed and returned to the branch office within an acceptable timeframe.

Schedule:

Ability to work Monday-Friday 8:30am-5pm variable schedule to include: days, afternoons, evenings, weekends as needed- Per Diem 18-22 hours per week

Must have a minimum of 2-3 days of availability to see patients per week (partial availability)

Must be able to drive your own properly registered/insured vehicle up to 60 miles one direction as needed (we offer mileage reimbursement and hourly rate of pay- even when driving!!)

Must be comfortable working in patients homes around animals/cigarette smoke

Must be knowledgeable of the following IV access devices to include: IV lines, accessing ports, and administration of infusion medications

Must have a single state RN license (or willingness to obtain) for the state of Ohio in good standing

Must live within 25 minutes of the Circleville, OH area

Requirements

• Current RN license by the applicable state’s Board of Nursing, in good standing. State of Ohio in good standing
• RN degree, bachelor’s degree in nursing (BSN) from an accredited institution preferred
• Advanced certification in specialty areas of practice (e.g., CRNI, ONS, IgRN) preferred but not required.
• Administrative Oversight: Responsible for providing administrative oversight, including monitoring of workflows, ensuring compliance with policies, and managing resources efficiently to meet organizational objectives.
• Emergency Availability: Must be available to respond promptly to emergencies, ensuring rapid and effective resolution of issues to minimize impact on operations. Availability during non-standard hours, including nights, weekends, and holidays, as required by the nature of the emergency.
• Minimum 3 years nursing experience with background in infusion therapy preferred.
• Experience in ICU and/or Cardiac Care preferred.
• Proficient with Microsoft Outlook and Microsoft Office Suite.Additional program knowledge of CPR+, Insight, Concur, Paylocity beneficial
• Valid state issued driver’s license with no significant restrictions on that license.
