Job summary

Metro Physical & Aquatic Therapy is seeking a motivated per diem Physical Therapist to provide in-home care in and around Westbury, NY. Flexible schedule and a focus on patient-centered outcomes are offered, with opportunities for mentorship and professional growth.

All levels of experience are welcome; pay ranges from $70 to $90 per visit, with 1099 and W-2 options available to fit your preferred work style. Licensure in NY or eligibility is required.

Qualifications

• Physical Therapist Program Graduate.
• Licensed Physical Therapist in the state of New York or eligible.
• Home health experience is preferred but not required - We will provide you with a mentor to help with documentation and compliance.
• Ability to travel to patients' homes within the assigned geographic territory.

Responsibilities

• Accept and treat patients within your designated geographic area.
• Evaluate patients' needs for physical therapy in the home setting and develop individualized treatment plans.
• Develop a plan of care and treatment goals in consultation with the physician, patient, and family.
• Plan, prepare and implement customized therapy programs to restore function and promote independence.
• Instruct family members, PTAs, Home Health Aides, and caregivers on appropriate therapy techniques.
• Provide timely and accurate documentation for each visit.
• Communicate effectively with patients, families, and interdisciplinary team members, especially around discharge planning.
• Deliver care in compliance with agency policies and professional practice standards.
