Metro Physical & Aquatic Therapy is seeking a motivated and passionate per diem Physical Therapist to provide in-home care in Western Suffolk County. The role is part-time with a flexible schedule, and the company offers both 1099 and W-2 options to fit your preferred work style.

Licensed Physical Therapist in New York or eligible is required; home health experience is preferred but not mandatory, as mentorship is provided for documentation and compliance.

Qualificações

• Physical Therapist Program Graduate.
• Licensed Physical Therapist in New York or eligible.
• Home health experience is preferred but not required; mentor provided for documentation/compliance.
• Ability to travel to patients' homes within the geographic area.

Responsabilidades

• Accept and treat patients within your designated geographic area.
• Evaluate patients’ needs for physical therapy in the home setting and develop individualized treatment plans.
• Develop a plan of care and treatment goals in consultation with the physician, patient, and family.
• Plan, prepare and implement customized therapy programs to restore function and promote independence.
• Instruct family members, PTAs, Home Health Aides, and caregivers on appropriate therapy techniques.
• Provide timely and accurate documentation for each visit.
• Communicate effectively with patients, families, and interdisciplinary team members, especially around discharge planning.
• Deliver care in compliance with agency policies and professional practice standards.
