Job summary

Metro Physical & Aquatic Therapy is seeking a motivated per diem Physical Therapist to provide in-home care in Eastern Suffolk County. The position is part-time with flexible scheduling and per-visit compensation.

All levels of experience are encouraged to apply; mentorship is available. We offer 1099 and W-2 options, opportunities for growth, and a supportive team of clinicians who focus on patient care.

Qualifications

• Must be licensed as a Physical Therapist in NY or eligible.
• Graduate of Physical Therapist program.
• Ability to travel to patients' homes within the geographic territory.
• Home health experience preferred but not required; mentorship provided.

Responsibilities

• Accept and treat patients within designated geographic area.
• Evaluate patients’ needs for physical therapy in home setting and develop individualized treatment plans.
• Develop plan of care and treatment goals in consultation with physician, patient, and family.
• Plan, prepare and implement customized therapy programs to restore function and promote independence.
• Instruct family members, PTAs, Home Health Aides, and caregivers on therapy techniques.
• Provide timely and accurate documentation for each visit.
• Communicate effectively with patients, families, and interdisciplinary team around discharge planning.
• Deliver care in compliance with agency policies and professional practice standards.

Education

Physical Therapist Program Graduate

Licensed Physical Therapist in NY or eligible
