Job summary

MultiCare Valley Hospital in Spokane Valley seeks a Hospital Social Worker to coordinate cross continuum care, perform psychosocial assessments, and implement interventions. You will identify opportunities to improve patient care, services and population health.

Requirements include a Master’s degree in Social Work, WA licensure (LICSW or LSWAIC with LICSW goal in 4 years), and at least 3 years of hospital or equivalent experience. Per diem schedule with varied shifts.

Qualifications

• Master's degree in Social Work required.
• Licensed in the state of Washington (WA).
• LICSW or LSWAIC with a commitment to obtain LICSW within four years.

Responsibilities

• Cross continuum care activities across care settings to improve patient care.
• Provide customer service to patient, family, and care team including discharge milestones.
• Maintain professional and proactive communication with colleagues and staff.
