Job summary

Constellation Health Services is seeking a per diem Licensed Hospice Social Worker to cover our territory in Rockland, ME and surrounding area. The role requires assessment of psychosocial needs for patients with terminal illness and collaboration with the interdisciplinary team to support care plans.

Responsibilities include education on advance directives, crisis intervention, and liaison with community agencies. Prior experience in home care/hospice is preferred and licensure is required.

Qualifications

• MSW accredited by CSWE or BS in social work.
• Meets personnel qualifications stated in §418.114(b)(3).
• Licensed driver with insured vehicle per state or org requirements.

Responsibilities

• Assess psychosocial status of patients and families.Maintain clinical records for all patients referred to social work.
• Educate on advanced directives and connect to resources.
• Provide crisis intervention when needed.
• Liaise with community agencies and interdisciplinary team.
• Assist in planning for funeral arrangements and end-of-life needs.
