Job summary

Pathways Healthcare, a leading home health and hospice organization in Massachusetts, seeks a LICSW to lead psychosocial aspects of patient care in our in-home program. You will conduct social assessments, develop intervention plans, and collaborate with a multidisciplinary team to support patients and families.

The role emphasizes discharge planning and resource coordination, with a focus on end-of-life care and maintaining thorough clinical documentation.

Qualifications

• LICSW required.
• End-of-life issues experience preferred.
• Expertise in psychosocial assessment and treatment.
• Collaborative approach to multidisciplinary team.
• At least 1 year of hospice care experience preferred.
• Strong verbal and written communication skills.
• Valid driver's license and own reliable vehicle.

Responsibilities

• Conduct psychosocial assessments related to patient illness and environment and share findings with the registered nurse.
• Develop social evaluations and intervention plans based on assessments.
• Maintain accurate up-to-date clinical records for all patients referred to social work.
• Provide information and referral services to patients, families, and caregivers regarding practical and environmental needs.
• Serve as liaison between patients, families, caregivers and community agencies.
• Build and maintain collaborative relationships with other healthcare professionals to support patient care.
• Work with public and private agencies to leverage available resources for patients and organization.
• Participate in development of comprehensive care plans and case conferences as required.
• Assist physicians and the multidisciplinary team in understanding social and emotional factors affecting health problems.
• Contribute to discharge planning and ensure smooth transitions for patients and families.

• Strong verbal and written communication skills, as well as good organizational skills.
• Possess a valid driver's license and own a reliable, insured vehicle.

Benefits & Compensation:

• Rates: $60 per visit + mileage reimbursement.
