Job summary

Altus Hospice is seeking a dedicated Licensed Master Social Worker (LMSW) to join our patient-centered team. You will deliver psychosocial support and counseling to hospice patients and families, coordinating with the RN Case Manager and care team to address emotional needs.

Per diem position covering San Antonio and surrounding areas, with 5-10 shifts per month. We offer comprehensive benefits and opportunities for growth, plus tuition assistance, PTO, and retirement benefits.

Qualifications

• Master of Social Work from an accredited school.
• Licensed Social Worker in good standing with State Board.
• Valid driver’s license, vehicle insurance, and access to a dependable vehicle.
• Preferred: 1 year hospice social work experience.
• Required: 1 year healthcare social work experience.
• State licensure required in TX (and listed states).

Responsibilities

• Assess psychosocial status and communicate with RN Case Manager.
• Evaluate medical needs, home situations, financial resources, and community support.
• Develop and implement personalized social work plans in collaboration with Bereavement Coordinator.
• Attend interdisciplinary team meetings and address psychosocial stresses.
• Provide empathetic counseling to patients and families.

As a Licensed Medical Social Worker (MSW) with us, your primary responsibility will be delivering exceptional social work services to our hospice patients and their families. Schedule: Per Diem, 5-10 per Month

• Be a Supportive Guide: Assess and communicate psychosocial status to the RN Case Manager and interdisciplinary group.
• Connect Hearts and Homes: Evaluate medical needs, home situations, financial resources, and community support.
• Provide Individualized Care Advocacy: Implement personalized social work plans and collaborate with your Bereavement Coordinator.
• Be part of a United Team Effort: Shape care plans, attend interdisciplinary meetings, and address psychosocial stresses.
• Provide Counseling with Compassion: Provide empathetic counseling to patients and families.

Qualifications

Education

• Required: Master of Social Work from an accredited school of social work

License Requirements

• Required: Licensed as a Social Worker from the State Board of Social Work
• Required: Current Driver's License, vehicle insurance, and access to a dependable vehicle

Experience

• Preferred: Minimum one (1) year experience as a social worker in a hospice setting
• Required: Minimum one (1) year experience as a social worker in a healthcare setting; long term care or medical surgical/acute care setting preferred

Additional State-Specific Requirements:

AL, AR, GA, IL, MA, ME, NJ, NM, OK, OH, RI, SC, & TX: Current unencumberedlicense as a Social Worker from the State Board of Social Work

$34.00 - $38.00 per hour (Average Pay Range). The pay range listed represents a general guideline for the role and is not a guarantee of the final offer. Compensation will be determined based on the selected candidate’s relevant experience and the specific responsibilities of the position. Final compensation rate will be discussed and confirmed at the conclusion of the interview process.
