Job summary

Constellation Health Services is seeking a dynamic Per Diem Hospice Registered Nurse to join our team in Rocky Hill, CT. You will provide skilled nursing care to hospice patients in their homes, assisted living facilities, and in skilled nursing facilities, coordinating with the care team.

The ideal candidate is a graduate of an accredited RN program, licensed in CT, with 1–2 years of RN experience and hospice knowledge.

Qualifications

• Graduate of an accredited RN program.
• Registered Nurse license in CT.
• 1–2 years RN experience; hospice experience preferred.

Responsibilities

• Provides skilled nursing care including medication administration as prescribed for home patients.
• Performs comprehensive assessments and coordinates care with the interdisciplinary team.
• Observes, evaluates and reports changes to the nursing supervisor and physician.
• Participates in weekly Interdisciplinary Team Meetings; communicates patient status.
• Maintains and processes documentation per agency standards; uses the electronic system.
• Provides input to Home Health Aide Supervisor for hospice aide evaluation.
• Weekend and holidays coverage as needed.
