Job summary

Transitions Hospice is seeking an experienced Registered Nurse for per diem hospice visits. This role supports the Extended Care Team across Columbus, OH, delivering skilled nursing care on a flexible schedule and collaborating with the interdisciplinary team to meet patient needs.

As an ECT Nurse, you’ll assess, treat, and document patient care with clinical precision. A current Ohio RN license, local travel, and adaptability are required for day-time per diem visits and weekend rotations.

Qualifications

• Active RN license in the state of employment.
• Graduate of an accredited school of Nursing.
• Clinical experience in hospice, home health, or a related setting preferred.
• Flexibility and confidence working across varied patient assignments.
• Strong assessment skills and sound clinical judgment under pressure.
• Excellent documentation habits and communication skills.
• Valid driver's license and willingness to travel locally.

Responsibilities

• Delivering professional nursing care across the full nursing process within your scope of practice.
• Conducting thorough patient and family assessments and contributing to individualized care plans.
• Completing all visit types as needed to meet patient and family needs.
• Administering medications and treatments per physician orders.
• Identifying nursing diagnoses from health assessment data and responding accordingly.
• Recording accurate, timely clinical notes including pain and symptom management outcomes.
• Communicating changes in patient condition to physicians and obtaining orders as required.
• Coordinating with community health partners to support seamless care transitions and arranging equipment/services.
• Maintaining and growing your clinical skills through continuing education and in-service participation.
