Transitions Hospice in Champaign, IL is seeking an experienced Registered Nurse for the RN Visit ECT role. You will provide skilled nursing care during visits, assess patient and family needs, and coordinate with the care team to ensure timely interventions.

This role requires adaptability, strong clinical judgment, and a willingness to travel locally. The ECT Nurse reinforces the care team to ensure no patient need goes unmet and to support families in challenging times.

Qualifikationen

• Active RN license in the state of employment.
• Graduate of an accredited school of Nursing.
• Clinical experience in hospice, home health, or related setting preferred.
• Flexibility and confidence working across varied patient assignments.
• Strong assessment skills and sound clinical judgment under pressure.
• Excellent documentation habits and communication skills.
• Valid driver''s license and willingness to travel locally.

Aufgaben

• Deliver professional nursing care across the full nursing process within your scope of practice.
• Conduct thorough patient and family assessments and contribute to individualized care plans.
• Complete all visit types as needed to meet patient and family needs.
• Administer medications and treatments per physician orders.
• Identify nursing diagnoses from health assessment data and respond accordingly.
• Record accurate, timely clinical notes including pain and symptom management outcomes.
• Communicate changes in patient condition to physicians and obtain orders as required.
• Coordinate with community health partners to support seamless care transitions.
