Transitions Hospice is seeking an experienced LPN for per diem, PRN visits in Columbus, OH. You will support the Extended Care Team across cases, providing skilled nursing care with speed and clinical precision to meet patient and family needs.

Formación

• Active LPN license in the state of employment
• Graduate of an accredited school of Nursing
• Clinical experience in hospice, home health, or related setting preferred
• Flexibility and confidence working across varied patient assignments
• Strong assessment skills and sound clinical judgment under pressure; excellent documentation habits and communication skills
• Valid drivers license and willingness to travel locally

Responsabilidades

• Delivering professional nursing care across the full nursing process within your scope of practice
• Conducting thorough patient and family assessments and contributing to individualized care plans
• Completing all visit types as needed to meet patient and family needs
• Administering medications and treatments per physician orders
• Identifying nursing diagnoses from health assessment data and responding accordingly
• Recording accurate, timely clinical notes including pain and symptom management outcomes
• Communicating changes in patient condition to physicians and obtaining orders as required
• Coordinating with community health partners to support seamless care transitions
• Maintaining and growing your clinical skills through continuing education and in-service participation
