Qualifications

• Current RN license in the state of practice.
• Active BLS certification for healthcare professionals.
• Minimum one year nursing experience; hospice or hospital preferred.
• Experience with an EMR/EHR system.
• Valid driver’s license and reliable transportation.
• Ability to communicate clearly in English (verbal and written).
• Flexible schedule to meet hospice patient needs.
• Ability to perform essential physical tasks including lifting.

Responsibilities

• Assess and identify patient/family needs using the nursing process.
• Coordinate the Plan of Care with the IDG for palliative care.
• Provide palliative and supportive care to patients and families.
• Document changes and communicate with IDG and physicians.
• Participate in admissions and educate patients about care requirements.
• Perform duties with compassion and maintain professional conduct.
