Job summary

UnitedHealth Group is hiring a Per diem Hospice Registered Nurse in Massachusetts to provide compassionate, patient-centered home-based care. You will assess needs, coordinate care plans, manage symptoms, educate families, and collaborate with the clinical team.

Responsibilities include initial nursing assessments, care planning, documenting progress, communicating with the care team, and offering on-call support.

Qualifications

• Current RN license valid for the state(s) you are assigned.
• Current CPR certification.
• Valid driver's license, reliable transportation.

Responsibilities

• Performs initial nursing assessment on new admissions.
• Coordinates and revises the total plan of care with continuity of patient care.
• Regularly communicates patient progress to the clinical team.
• Develops and maintains individualized patient care progress records.
• Educates families on patient condition and care needs.
• Performs on-call responsibilities and on-call services as assigned.
• Performs other duties as assigned.
