Legacy Hospice is seeking a compassionate Registered Nurse Case Manager (RNCM) for per diem coverage in the Richmond area. The role focuses on hands-on hospice nursing, care coordination, and guiding patients and families through the hospice journey.

You will join an interdisciplinary team delivering care grounded in compassion, clinical excellence, and connection, with weekday scheduling and per diem flexibility.

Qualifications

• Hold current unencumbered license as a Registered Nurse.
• Minimum one (1) year experience as an RN in a medical surgical/acute care setting; hospice exp is a plus.
• Must be computer proficient, including EMR proficiency.
• Maintains CPR certification if required by state.
• Current driver's license, vehicle insurance, and dependable transportation.

Responsibilities

• Empower Through Engagement: Respond to clinical referral information with care and expertise, addressing inquiries and requests with a compassionate touch.
• Collaborative Care Champion: Partner with Clinical Director to assess eligibility for hospice services and ensure appropriate care at the right time.
• Nurturing Direct Care: As an RN Case Manager, coordinate care while ensuring patient comfort and quality of life.
• Nursing with Heart: Deliver top-notch care rooted in proven nursing principles and techniques.
• Educator and Advocate: Educate patients and families, empowering active participation in care plans.
• Call of Compassion: Share in call duties, demonstrating commitment to being there when it matters most.
