Job summary

Provident Behavioral Health seeks a Hospice Nurse (Home Care) to provide patient-centered hospice nursing in patients' homes. You will assess admissions, develop and coordinate individualized care plans, and manage symptoms to support comfort and quality of life.

You will collaborate with physicians and the interdisciplinary team, document observations, and educate families. On-call duties may be required to ensure continuous, compassionate care for patients and their loved ones.
