Job summary

Black Women's Mental Health Institute is seeking a compassionate hospice professional to conduct holistic assessments and coordinate individualized care plans for patients and families across physical, psychosocial, and spiritual domains.

You will collaborate with physicians and interdisciplinary teams to ensure coordinated, patient-centered treatment, document care, and educate caregivers while supporting timely clinical decisions for home or facility hospice services.

Qualifications

• Ability to perform holistic assessments across physical, psychosocial, and spiritual domains (must demonstrate critical thinking).
• Strong care planning capabilities with patient-centered goals and measurable outcomes.
• Proven ability to work within interdisciplinary teams to coordinate treatment.
• Experience educating caregivers and delivering compassionate interventions.
