Job summary

Hartford HealthCare at Home is seeking a Per Diem Hospice Physician to provide medical oversight, clinical consultation, and hospice services in collaboration with the interdisciplinary team in Hartford, CT.

This flexible per diem role offers an hourly pay of $150 and opportunities to work with our dedicated hospice staff while delivering compassionate, patient-centered care to individuals and families at the end of life.

Qualifications

• Medical degree and Connecticut medical licensure.
• Experience in hospice/palliative care.
• Strong communication and teamwork skills.

Responsibilities

• Certifying and recertifying patients who meet hospice eligibility requirements.
• Participating in Interdisciplinary Group (IDG) meetings and contributing to plans of care.
• Providing clinical oversight to hospice staff, including RNs and APs.
• Guiding on pain and symptom management and writing medical orders.
• Reviewing and approving plans of care for compliance and quality.
• Collaborating with attending physicians to ensure continuity of care.
• Reviewing clinical documentation and patient records for quality and regulatory compliance.
• Providing home visits, in person or virtual, when appropriate.
