Qualifications

Responsibilities

• Perform skilled nursing assessments and deliver care in patient homes.
• Develop and adjust care plans with client and interdisciplinary team.
• Communicate with physicians, coordinate services, and document care.
• Provide patient/family education and support to promote comfort and quality of life.
• Lead and supervise care teams and manage referrals and follow-ups.
