Hartford HealthCare at Home is seeking a Per Diem Hospice Physician to provide medical oversight, clinical consultation, and hospice services in collaboration with the interdisciplinary team in Hartford, CT.

This flexible per diem role offers an hourly pay of $150 and opportunities to work with our dedicated hospice staff while delivering compassionate, patient-centered care to individuals and families at the end of life.

• Collaborating with attending physicians to ensure continuity of care.
• Reviewing clinical documentation and patient records for quality and regulatory compliance.
• Providing home visits, in person or virtual, when appropriate.

As a Per Diem Hospice Physician, you will have the opportunity to contribute your clinical expertise to an interdisciplinary team while helping patients and families navigate some of life’s most important moments.

The Opportunity

In this role, you will provide medical oversight, clinical consultation, and hospice medicine services in collaboration with hospice physicians, advanced practitioners, nursing professionals, and other members of the interdisciplinary team.

Responsibilities include:

• Certifying and recertifying patients who meet eligibility requirements for hospice care.
• Participating in Interdisciplinary Group (IDG) meetings and contributing to individualized patient plans of care.
• Providing clinical oversight and consultation to hospice staff, including registered nurses and advanced practice providers.
• Offering guidance on pain and symptom management and writing appropriate medical orders.
• Reviewing and approving plans of care in accordance with applicable requirements.
• Collaborating with attending physicians to promote continuity of care and ensure patient-centered treatment decisions.
• Reviewing clinical documentation and patient records to support quality care and regulatory compliance.
• Providing home visits, in person or virtually, when clinically appropriate.
• Supporting the ongoing development and delivery of hospice services in collaboration with the interdisciplinary team.
• Maintaining accurate records of services provided, including dates of service, the general nature of services performed, and time spent.

Qualifications
