Frontier Hospice is currently hiring Per Diem Hospice Nurse Practitioners to deliver compassionate patient care in and around the Lawton area. You will evaluate health histories, diagnose conditions, and create personalized care plans in collaboration with patients, families, and the healthcare team.

Qualifikationen

• Master's degree in nursing or related field is required or preferred.
• Unencumbered RN/NP license in the practicing state.
• CPR certification may be required by state.

Aufgaben

• Deliver comprehensive hospice medical care, palliate, and manage terminal illnesses under physician supervision.
• Collaborate with attending physician and hospice medical director for eligibility and ongoing care needs.
• Coordinate patient care with the interdisciplinary team to ensure holistic, patient-centered care.
• Educate and counsel patients and families on care goals, prevention, and spiritual aspects.
• Provide input on patient eligibility for initial certification and recertification for hospice care.
• Maintain up-to-date knowledge of Medicare/Medicaid and state/federal hospice regulations.
