Job summary

UP Health System Hospice, part of the Optum family, is seeking a Per Diem Licensed Practical Nurse (LPN) to provide quality patient care in patients' homes. The role requires nursing skills under RN supervision, medication administration, and support for physicians and families in a hospice setting.

The position emphasizes compassionate care, wound management where needed, and helping patients with daily activities while addressing changing conditions at home.

Qualifications

• Current and unrestricted LPN licensure in state of practice.
• Current driver’s license and vehicle insurance, and access to a dependable vehicle or public transportation

Responsibilities

• Performs skilled nursing services under the supervision of a staff RN daily.
• Administers prescribed medications and treatments as permitted by state regulations
• Assists the physician and/or staff RN in performing specialized procedures as needed
• Assists the patient with daily activities and encourages appropriate self-care daily
• Reports to the patient's physician and other appropriate personnel regarding pertinent changes in the patient's condition as needed
• Performs complex wound care if in-service is documented for specific procedure daily or as needed
• Ability to function in any home situation regardless of age, race, creed, color, sex, disability, or financial condition of the client
