Job summary

Protouch Staffing is seeking experienced Per Diem LPNs for hospice and home care in the West Palm Beach area. The role focuses on Continuous Care with 12-hour day and night shifts, primarily Friday through Monday, offering flexible scheduling based on patient needs and staffing.

Ideal candidates have strong hospice experience, HCHB proficiency, and the ability to work independently in home settings, with travel within 50 miles of West Palm Beach, FL.

Qualifications

• Current Florida LPN license in good standing.
• Current BLS certification.
• Minimum of 1 year of current, applicable nursing experience.
• Strong hospice experience required.
• HCHB/Homecare Homebase experience required.
• Graduate of an approved practical nursing program.
• Ability to travel within a 50-mile radius of West Palm Beach, FL.
• Ability to work independently in a home/hospice environment.
• Must be a U.S. Citizen.

Responsibilities

• Provide high-quality, patient-centered nursing care in the home and hospice setting.
• Provide Continuous Care during assigned 12-hour shifts.
• Build meaningful, compassionate relationships with patients and their families.
• Provide nursing services according to the patient's individualized plan of care.
• Follow physician orders, agency policies, and applicable state and federal regulations.
• Observe patients for changes in condition and report adverse reactions to medications or treatments to the appropriate clinical team.
• Assist the physician and/or Registered Nurse with implementation of the patient's plan of care.
• Perform nursing procedures and specialized treatments within the LPN's scope of practice.
• Educate patients and families regarding medications, diet, treatments, and appropriate care.
• Accurately document patient care, interventions, and clinical observations in HCHB.
• Communicate effectively with the interdisciplinary care team.
• Maintain professional standards and compliance with agency policies and regulatory requirements.
