Job summary

Empath Health is seeking a Continuous Care LPN to provide bedside clinical observations, prescribed care, and emotional support to patients in a hospice setting across Manatee, Charlotte, DeSoto, and Sarasota counties.

You will work under the RN case manager, process physician orders, and assist with overnight triage when needed. Florida LPN license and acute care experience are required, hospice experience preferred.

Qualifications

• Graduate of an approved nursing program and hold a current Florida LPN license.
• Minimum one year of experience in an acute care setting providing direct patient care; hospice LPN experience preferred.

Responsibilities

• Under the direction of the RN case manager, provides continuous bedside clinical observations, prescribed care, physical and emotional support to patients as directed in plan of care, reporting significant changes in patient condition and family needs as needed.
• In collaboration with the RN, accepts and processes physician orders to maximize patient comfort.
• Continuous Care LPNs assist Triage and Care Centers overnight when Continuous Care cases are pending.
