Job summary

Holisticare Hospice is seeking a Per Diem Hospice Aide (CNA) to join our compassionate team, delivering hands-on end-of-life care with respect and empathy in the Wyomissing area.

Responsibilities include following care plans, assisting with personal care, maintaining thorough documentation, and supporting families and the nursing team. Requires CNA certification, CPR if mandated, and reliable transportation.

Qualifications

• High School Diploma or GED required.
• Current unencumbered CNA certificate.
• Minimum of 1 year CNA experience in acute or long-term care; hospice experience a plus.

Responsibilities

• Provide personalized care following the care plan and respond promptly to patient and family requests.
• Assist with personal care needs (bathing, oral hygiene, bedding, dressing, grooming) to maintain dignity.
• Demonstrate exceptional customer service and patient satisfaction.
