Crescent Hospice is seeking a Per Diem Hospice Aide (CNA) to support patients at the end of life. Working with our nursing team, you will provide hands-on care with dignity and compassion in patients' homes and community settings.

The role requires a valid CNA license, CPR if required, and at least one year of CNA experience in acute or long-term care. A valid driver's license and reliable transportation are essential.

Qualifications

• Requires CNA certification in the state.
• Minimum one year of CNA experience in acute or long-term care; hospice experience is a plus.
• CPR certification is required if state mandates.
• Must have a valid driver's license and reliable transportation.

Responsabilités

• Provide personalized patient care according to the plan of care.
• Assist with personal care tasks to maintain dignity and comfort.
• Deliver compassionate, responsive support to patients and families.
• Maintain accurate documentation in patient records and agency systems.
• Collaborate with nursing team to ensure quality end-of-life care.
